First Visit

Please come early, even though the office is conveniently located off I-81, most people are nervous and don’t see the sign for the Lackawanna Executive Office Park.

Park in the spaces designated for Dr. Zale in front of the office.

Please complete medical questionnaire and dental history form (yellow sheet) both sides.

* Bring X-rays from general dentist. (This office requires a written release authorization to request x-rays from another office.) The insurance company requires a full set of xrays to approve special treatment for the gums. Their definition of a full set is 14-18 small xrays or 1 large panoramic survey with 2-4 small bitewings. Pleae contact your general dentist to ask if a full set was done within the last 5 years. If not, please have him/her take them, then send or bring them with you. The insurance company is clear about the number thy want – not just 2-4 small xrays. Instead a full set is considered 14-18 small xrays or 1 large panoramic survey with 2-4 small bitewings.

  • Insurance card.
  • Valid drivers license.
  • List of prescription medications.
  • Questionnaire, if downloaded from the web page.
  • Eyeglasses, to read information.
  • Sweater, due to cooler temperature in the office due to the staff’s water resistant garb.

A proficient understanding of English is necessary for the patients informed consent for any exam or treatment in this office. (Sometimes an interpreter is needed to better convey the important dental terms.)

Don’t worry, the exam starts easily, and Dr. Zale will stop any time you want.
